6. Patient Responsibilities
If you receive care from Woundworx, you agree to:
• Provide accurate, complete information about your health, medications, and history
• Notify us of changes in your condition, contact information, or insurance
• Follow your treatment plan or notify us if you cannot
• Be present and prepared for scheduled visits
• Treat our clinical team with respect
• Pay your portion of fees in accordance with your insurance benefits
7. Cancellation and No-Show Policy
We require 24 hours notice for appointment cancellation. Repeated no-shows or late cancellations may result in a no-show fee (where permitted by your insurance and state law), removal from regular scheduling, or discharge from the practice.
8. Billing and Payment
Woundworx accepts Medicare, Medicare Advantage, and several commercial and Medicaid plans. Specifics are listed on our website and verified at intake. You are responsible for any deductibles, copays, and coinsurance under your insurance plan; services not covered by your insurance (when you have been informed in advance); and out-of-pocket payments for services elected outside of insurance. We will provide a Good Faith Estimate for self-pay patients in accordance with the No Surprises Act.
9. Affiliated Entities
Woundworx LLC is part of a portfolio of affiliated healthcare entities. We may coordinate care with The Rest Rx (DMEPOS supplier) and Care Connection Home Support (RSA Level 3 personal care agency). When we refer or coordinate with affiliated entities, we disclose the affiliation and you may choose alternative providers at any time.

All content on thewoundworx.com — including text, graphics, logos, images, and software — is the property of Woundworx LLC or its content suppliers and is protected by U.S. and international copyright law.
